What services am I able to receive if I am 12yrs or older?

>> Periodic Exam: once per plan year for new patients OR twice per plan year for continuing patients
>> Full Series X-rays every five years
>> Bitewing X-R\rays every four years
>> Professional Cleaning twice per plan year
>> Fluoride twice per plan year
>> All other dental procedures, no limit during the plan year and a 20% discount on regular fees (Specifically excludes procedures not performed in our practice, i.e. referrals to specialists)
* New patients enjoy first-year savings of up to $136 PLUS 20% off all other in-office services PLUS $50 Renewal Bonus.
