What services am I able to receive if I am 12 yrs or older?
>> Comprehensive Exam for new patients-only
>> Periodic Exam: once per plan year for new patients OR twice per plan year for continuing patients
>> Full Series X-rays every five years
>> Bitewing X\rays every four years
>> Professional Cleaning twice per plan year
>> Fluoride twice per plan year
>> All other dental procedures, no limit during the plan year and a 20% discount on regular fees (Specifically excludes procedures not performed in our practice, i.e. referrals to specialists)
What services is my child able to receive if they are 6 yrs old to 11 yrs old?
